Two sex offenders were killed in a 10-hour riot at an understaffed maximum security prison, a report has found.
Several hundred inmates at Tecumseh State Correctional Institution, Nebraska, went on a rampage after being let out of their cells to get over-the-counter medication around 2am on May 10.
Two prison officers were also assaulted in the incident and one inmate was shot in the leg.
A report by the Nebraska Department of Correctional Services described the riot as a 'destructive 10-hour rampage'.

A summary stated: 'The uncontrolled inmates assaulted two staff, seemingly ignored warning shots and actual use of deadly force, and essentially entrapped and taunted numerous staff members in buildings overrun by inmates.'
Inmates were also reported to have caused thousands of pounds worth of damage as they lit fires, broke down doors and windows, and trapped staff.
They also chanted 'Hands up, don't shoot', a phrase used in the Ferguson protests last year after unarmed teenager Michael Brown was shot dead by a police officer.
Nineteen staff - out of the 57 on duty - and one volunteer had to be rescued after authorities arrived at the prison around midnight.
The report continued: 'The staff endured situations that one cannot always prepare for, yet continued to stay alert and aware.'
Scott Frakes, the director of the Nebraska Department of Correctional Services, said the report provides a realistic assessment.
He said: 'In addition to identifying needed improvements, the report also highlights the courageous actions of staff and in the dedication shown since the disturbance.'
